It would be helpful to know the game, and the patch you want to apply to it...
Which game ?

What patch ?

Where did you download the patch from ?

More info is needed before anyone can help you.

What form does the patch come in? Exe-File? Setup? In that case simply start it, and see what happens.
Zip-Archive? Open it and look for a readme file.

That's rather an obscure game. You are better off asking in the forum where you downloaded the patch from.

In most cases, the patch are exe which you just double click to run. Point it to your install directory if it asks. If its a zip file, unzip into a separate folder and look for any readme or txt file which might have instructions in it.

If either fails the better ask in the game forum.
I have that patch too unsure how to use it since i don't have the game yet.
First it would be good if you unzip the zip or rar file with something like winrar or 7zip.
And then just replace the files in the main installation
It actually comes with folder MaidensOfMichael-2.0-adult_patch containing game folder and with files archive_ex.rpa and script_ex.rpa

And a game only folder with the same files but different dates, i bet you only need to use the adult patch files though not sure.
Strange that there is no read me file in the zip.

Edit: Btw after checking some more info didn't find out how to patch it but is Runa really missing from the game or her scenes even with the adult patch?
Read it was quite an annoying character though, but that was only a comment.
Needs further explanation on this.

I don't have the game - but my guess it to extract the files from the archive file you downloaded into the game's directory. Run the game then look for any signs that the patch worked - a note on the title screen indicating the game has been uncensored / extra spots in the CG and/or events gallery.

That *should* work - if not you may need to move the files to a subfolder - since I don't have the game I can't check that for you
Oh i also found this:

The all-ages version does contain modified content from the adult version. Customers who wish to have the full and uncut Maidens of Michael experience can download the free adult content patch on launch, and enable said content from the “config” menu once the patch has been applied.

So apply patch and use the config menu then, though this was for the steam version and the game got removed from steam.
Maybe the config option applies to gog also?

1. Download a program to open archive files - I recommend 7zip (http://www.7-zip.org) but WinZip or WinRAR will work as well

2. Open this program and use it to open the file you downloaded from mangagamer

3. Drag the files from 7zip et al to the directory where the executable for the Maidens game is

4. Run Game

Heh, i have a familiarity with working with the rpa files. Drop those files in the 'game' folder, which is where the other rpa files are (one could be archive.rpa), far as i can tell that should be it, unless you need to replace the exe.

hnae003:
I also downloaded it from there, it says type of file winrar zip archieve. Anyway did you try extracting the files yet?
You should try extracting them and copy them to C\gog games\maidensofmichael or where ever you installed the game.
You need to replace the files in the folder where youi installed the game with MaidensOfMichael-2.0-adult_patch folder inside you will find archive_ex.rpa and script_ex.rpa and copy them to the game installation folder with the same names of files and replace them after that look for config menu if it has any settings to change it to adult version.

Btw i don't know how to properly quote posts sometimes so i do it like it is. If i don't properly quote posts my posts on gog won't go through.

Edit:Sorry i forgot to say the config menu is inside the video game menu you will need to run the game when you properly patched it..
