Fair warning before reading: might be minor spoilers.

So, I have come into a problem. Surprisingly, googling around, there's apparently only two people on the internet who have had this bug (or at least thought to ask about it). I missed "The Last Wish" my first time through, so just this morning, I reloaded an earlier save (right before doing "A Matter of Life and Death" and "Now or Never" and before going to Skellige), and went through all the main story quests (and did a few sidequests, like Triss' last ones, and Hjalmar and Cerys' quests), not romancing Triss and romancing Yennefer.

I got to "The Last Wish", and it all went well. I could complete the quest and got the cutscene at the end with Geralt and Yennefer on the ship on top of the mountain, returning to Kaer Trolde where they said they'd have to go off to see Emhyr.

I went to Crow's Perch and went to go take Uma, but as soon as I go into the stables to retrieve Uma, the game tells me that "The Last Wish" failed. I reloaded a save I made right before going to Uma, in the yard in Crow's Perch right in front of the stables, and it says "The Last Wish" is Completed. As soon as I initiate the cutscene, it goes from Completed to Failed.

So...seriously. What gives? Has anyone had this and found a way to fix this? Has anyone had this bug but gone on with the game and still successfully romanced Yennefer in the end (and gotten an ending where Geralt's with her)? Any help would be appreciate with this absolutely ploughin' ridiculous bug. I spend hours to get to this point, just 'cause I didn't want to even accept the possibility of an ending without her.

I've experienced a bug where a completed quest pops up as failed later on, only mine was an assassination quest. At the end of the game, though, things played out like the quest was completed successfully, so there's a significant possibility that it being randomly marked as failed doesn't "count" in the game if you completed it. That's a guess based on my own experience, of course, but it seems like it's the same bug.

"The Last Wish" is a buggy, buggy quest. If you get out of the boat at the last stop before it's reached the appropriate point and sit down on the boat once you get back in (the cutscene doesn't trigger immediately since the boat stops when you get out and thus isn't in the right position), you either get an endless loading screen or the graphics will freak out, teleport Geralt to shore, mark the quest as failed, then lock up. It was intense the first time that happened.
I reloaded the save and re-did the quest, this time disabling the console by deleting the console enabler files while I did the quest. Surprisingly, this time the quest didn't fail, and I wasn't given the option to apologize to Yennefer at Vizima after the quest. It seems it was fixed, so I can just recommend to disable console while you do the quest (or even just disable it altogether).
Try disabling alternative look for Yennefer, that did the trick for me.

I got this same bug. Read that 2 mods can cause it: fast travel and debug console.

Disabled the first didnt fix the issue. The debug console mod had 3 files: dsound, debug and telemetry. Deleting the first 2 dont fix things, deleting the last causes script errors.

Kinda stuck atm >.<
Ok. After reviewing many threads from Google. I gave it another attempt. Not sure which of the following did the trick, but I tried all with a save before contacting the sergeant at Crow's:

1. Savegame before speaking with sergeant;
2. Disabled and deleted fast travel anywhere mod;
3. Deleted debug console file and plugins from bin folder;
4. Did a random quest in Velen;
5. Used normal signpost to travel to Skellig;
6. Travelled back to talk to sergeant re Uma.

This time, no The Last Wish failure notice. I'll have to remember these for the second playthrough =S

Ugh... I agree with you. I just finished "The Last Wish" quest and after I found Uma, I ran into the same issue as you did :-(

Really frustrating...

So, I Googled the issue and found several posts on the subject and they all seem to point to a mod or Yen's alternative costume.

I dunno.... I've disabled all the mods and just reloaded a save to try again.

-I disabled the fast travel mod.
-Disabled the enhanced graphics mod.
-I disabled a mod that changes the color of Roach.

I reloaded my save from the point where I finished "The Last Wish" quest and somehow, that seemed to solve the issue.
