BUGS
--Mutagens and some general skills (e.g. Griffin School Techniques) must be replaced in the character screen after each game load in order for their bonuses (e.g. to attack power, sign intensity, etc.) to be applied properly.

--Buying the Enhanced Moon Dust and Enhanced Northern Wind manuscript pages doesn't actually add the recipes to your alchemy list.

--Chernobog rune stones don't work as intended; they display upgrades of "0 Attack."

--All merchants that have been "discovered" should have their icons permanently visible on the main map, unless they've somehow been removed due to completing a quest.

--The alchemist just outside Novigrad to the north of Oxenfurt Gate buys and sells all items at face value, and is the only merchant I've found (after 40 hours of playing) who buys crossbow bolts. The buy/sell value "exploit" should be fixed, while all blacksmiths in the game should be able to buy crossbow bolts.

--Some merchants (e.g. the blacksmith in the NE corner of the Novigrad Fish Market, the alchemist just outside Novigrad to the north of Oxenfurt Gate, etc.) aren't always present/visible when they should be; meditating "fixes" this.

--Some quest items (e.g. the "Cage Key" used in a "Person(s) in Distress" location - I forget which one exactly, one "Squirrel Tail" used in the "Contract: Woodland Beast" quest, the "Silver Monocle," etc.) aren't correctly recategorized as non-quest/sellable items after the related quest is completed.

--Some quests (e.g. the "Deserter Gold" treasure hunt quest in White Orchard, the "Coast of Wrecks" treasure hunt quest in Farcorners, etc.) aren't added to your quest journal if completed in a certain order.

UI SUGGESTIONS
--The option to disable autosaves/checkpoint saves should be added.

--Mechanics allowing storage of items in containers should be added.

--Swords should never be "auto-sheathed"; swords should only be sheathed when the player does so manually.

--Herbs should be harvested without the need to open a loot window.

--The popup window that appears when hovering over each item in a merchant's inventory (with the exception of equipment) should also display how many of that item you already own.

--Inventory item sorting could be improved; in particular, it should be much easier to see how much alcohest is in your inventory at any time. Also, alchemy items, crafting items, and books/documents should have separate tabs.

--Books/documents should be marked as "read" or "not read" in all inventory screens.

--Containers that are near/attached to light sources should be made easier to loot - i.e. without the "Ignite"/"Extinguish" prompt getting in the way.

Added a couple more suggestions. (Autosaves can get pretty ridiculous.)
BUG:
Mutagens dont work properly. The amount of % increase is incorrect after loading a game. After every loading you must replace them to have proper stat increase.

Thanks - I hadn't noticed that until you mentioned it. Added to the list.
Thanks for writing this down you saved me a lot oftime since i was about to do the same.

But I also like to add a few things:
I did not notice any changes within the game after I helped killing Radovid. Might be bug.

Rosh gave me a warm welcome but i did start the game with a Witcher 2 Savegame where I helped Iorveth and let the Kingslayer go so i did not expect a warm welcome. Maybe a bug or a fail during Savegame import.

During the Quest to kill Radovid he told me that he hopes i had a better prove than the last time but within the sidequest with Phelippa i did not show him that kristal and gave it to Triss.

One of the Muhmen took the Witcher Medallion from Ciri but she still had it later on without getting it back within any quest.

Note: I played the german Version in case it matters.

--Chernobog rune stones don't work as intended; they display upgrades of "0 Attack."
THIS!
As for merchants missing it might be way to early or too late that is why they are not present.

The "missing" merchants bug with these particular merchants actually happens during the day when they should definitely be present, and I tested thoroughly - e.g. at 10AM the blacksmith wasn't at his forge; I meditated 1 hour and he suddenly appeared. To make sure he was "supposed" to be there at 10AM in the first place, I left Novigrad, came back for a while, and returned at 9AM, then meditated for an hour until 10AM. He appeared again.

Well never had this issue.

Thanks - I haven't played that quest yet myself (and I'm also playing the English version), but good to know!
How about adding a specific button to take in-game screenshots? I have tried everything I could but could not take any in-game screenshots!!! I wanted to show `invisible` horses being ridden and invisible horses riding through the soil/road and sometimes only heads of the riders showing above the road as rest is buried or should I say riding through the road...sometimes the horse and rider floats in the air! Has anyone figured out how to take a screenshot? Repeat: normal pc screenshot buttons do not work. These are times when I wish I had this game on Steam: it never fails to take screenshots!!!

And the mutagen bug also seems to affect general skills as well; updated the list. Again, pretty huge/annoying issue - hopefully will be fixed in the next patch.

Rozenman - would you mind checking out those two merchants and letting me know what you find? You probably just didn't notice the issue with them, as they're the only two I've found so far that seem to have this problem (though I'm only halfway through Velen/Novigrad).

mad2gamer - wish I could help with the screenshot issue, but I haven't tried myself. Maybe someone else has an idea?
There was that one quest in Skellige where I had to kill a giant and there was this guy in the cage since there is no real stelth the giant woke up whatever I did. I was not able to get the key without him waking up! During combat I was able to take the key but there was no activator to open the cage during combat.

They also could add an option to disabel the "last time on Witcher 3" thing every time you load the game that is sometimes realy anoying.

Yeah, I (and many others here and on the Steam forums) agree on this. I'd rather at least have the option to view the gameplay tips during load times instead of the same story recaps over and over (and over and over and over).
The new loot window is way to big.

And I just rememberd That there was this one bug that Triss told me of some torturering, but I never let the witchhunter ley a finger on her during that quest where she was offering herself as a bait.

Also have you seen the bug from this vid yet?
Vid: http://www.gamestar.de/videos/candyland,102/the-witcher-3,82844.html
It's about the head and animation bug.
Shadow - yeah, I have seen that bug from the video a couple times myself (once in Novigrad, and once in some village in Velen I think). Walking through a cloud of flying human heads was... disturbing.
