I know that I need to travel to the mere of dead men to forge the sword of Gith, but I'm stuck. My map no longer shows West Harbor or the nearby ruins. I've revisited every spot on the map looking for clues and have come up empty. What am I missing?

You don't travel via the map - when you reach the right part of the story (after visiting Nolaloth) you can access a teleporter in the Arvahn Ruins to travel to West Harbour.
