Virtua Fighter 2 - Akira's stage mainly for his theme music.
Street Fighter Alpha - Dramatic Battle.
Metal Gear Solid - Psycho Mantis.

Oh jesus, and then when you came back after finishing an objective or something one of the flying necros would be turning all the corpses into super necromorphs. Awful.
Stage 3 of Castlevania: Dracula X if only because it was the first time I heard "Bloody Tears." Still probably my favorite in-game music ever.
Company of Heroes had a few: Carentain: Counter Attack, Chambois & Caen: Into the City
STALKER: Dark Valley Secret Lab

Unreal Tournament: CTF Facing Worlds
The final level of Supreme Commander from all faction's perspectives.

Oh yes REZ. First time I loaded this on my Dreamcast I was stunned.
VTM: Bloodlines- the haunted hotel (nearly had me jumping out of my skin)
Psychonauts- Lungfishopolis and the Milkman Conspiracy

Well designed and varied enough so that you have to use a good part of your gaming skills (alternate shooting+shielding, platforming, timing and so on) to get it through with fewer flaws in your play.
>> Portal (PC) the "level" after the 19 starting trials - This is just fucking awesome :-P
>> Half-Life 2 beginning - I was simply shocked when I first saw City Seventeen with my bare eyes....
>> Prince of Persia: The Sands of Time (PC) the "Aurora Tower" run - I've played this countless times, and I still find it worth a bit of my day from time to time. Being unable to use the dagger to rewind time, the Prince must be very careful not to waste his platforming and climbing and fighting. And the end, once again, is fucking awesome :-D

Jedi Knight ---> That falling level. I'd never seen anything like it before. I think it's still the best implementation to date.
All those I am going to mention I think has already been mentioned once before but they are awesome, so they deserve to be jotted down again.
Half-Life - Blast Pit -- Challenging, fun, and it also shows how quickly the Xen have taken over Black Mesa and installed themselves and how powerful they are. I love this level even though I died a gezillion times when I first tried to beat it.
Half-Life 2 - Ravenholm -- So unsettling, so atmospheric, very well-designed level. Perhaps the best in the game itself and the future episodes, I really enjoyed Ravenholm (if not mostly because of Father Grigori, the classic 'no-fear' mad priest character). The screams of the Fast Zombies are bone-chilling the first time you go through the level.
Vampire Bloodlines - Grout's Mansion -- Spooky and unsettling, but not horrifying like earlier in The Hotel level, Grout's Mansion just does it every time for me. The atmosphere is great and has really been paid alot of attention in developing this level and I really love it, this is definetely the best out of the entire game.
Psychonauts - Lungfishopolis -- The level I use constantly to try and convince people to try and buy this game (if they haven't already), because it is hilarious. Turns everything upside down and let's you live out that Godzilla fantasy you've always had.
Serious Sam - The Great Pyramid -- Just one big complete 'WOW!' moment. An epic level, nothing more be said. It is even better in the new HD version of the game!
