I have at this point duplicate games in my Game library. I have a couple of Wolfenstein 3D Entries. one is 12MB the other is 18MB. All version 1.4. Also the number of games in my library does not match the real number. Wolfenstein 3D and Spear of destiny jumped to the start of the list when sorted by Purchase date. I own these games for years. In the history of my purchases it says I bought them on the 11th of August 2023, this is not true.

What is going on?

Quite odd about the 11th of August, 2023. It should have been 25th of January 2023.

Bethesda removed 2 mission packs for Spear of Destiny and re-released Wolfenstein 3D together with Spear of Destiny (you can now select it in game menu), therefore old entries for Spear of Destinty and Wolfenstein 3D were removed in favour of that new one. GOG users were not happy about it and GOG decided to bring those back for original owners, hence you have 3 entries instead:

Wolfenstein 3D (+ Spear of Destiny, but without mission packs)
Wolfenstein 3D (removed and later re-added, original campaign)
Spear of Destinty (removed and later re-added, +2 mission packs)

Yes it was exactly this, they have different versions. And as someone mentioned as well, the game count went up by 2. Which is my case as well. Perhaps this will be fixed soon.

It will not be fixed, because it's fixed already. There is no other way to bring old entries back, logically it would be just to add old Spear of Destiny (with 2 mission packs) but not Wolfenstein 3D, the problem is that those 2 are inseparable.

Therefore you have 2 entries for Wolfenstein 3D (old and new) and not just Spear of Destiny.
