i want a game that allows for choices to be made and matter or affects the over all game

not games like draonage 2 were choices still result in the same outcome
but something like detroit become human

also would like games that lets you improve relations with other npc *does not have to include romance*

For choices that matter:
* In Dragon Wars, it's possible for some places to become destroyed or abandoned depending on your actions. Also, many (adventure game-style) puzzles have multiple solutions, especially the task of escaping Purgatory, where you start.
* Wasteland also has an instance where a town can be abandoned. Wasteland 2, from what I've heard, has a part early on where two areas are under attack, and you can only save one of them.
* Stranger of Sword City Revisited has you choosing which divine vessel you want to give blood crystals to, and the one you give the most to will determine which ending you get, and will also affect which final bosses you fight.

Improve relation with NPCs:
* Saviors of Sapphire Wings: While they're actually party members rather than NPCs in the usual sense, you can improve your relation with them by fighting with them, having meals together, and some dialog choices will improve or worsen relation. (In fact, you are *required* to improve your relation with Eltha and Lumi to a certain level in order to complete the game.)

Would the KotORs fit? Or Arcanum?

Piranha Bytes games might also fit, though not really for the NPC relations part, as there it's mainly factions.

Good CRPGs like Fallout and Pathfinder would be your best bet there. If you want something more "AAA" then Fallout New Vegas is probably the best action RPG for it, off the top of my head. Maybe the Piranha Bytes games like Risen and Elex, though that's more about faction choices than smaller decisions usually.
I remember playing the demo of Castles 2, and periodically you'd get choices of helping the princess or not, etc etc. In one game on the demo i married her like 5 times. In some choices if you do them wrong you'll get beheaded or attacked.

Though what the full game is like i am not sure, i played the demo back in 1995. (It was on the Battlechess CD, Battlechess took like 100Mb, so they just shoved demos on it, including lord of the rings, and a few other titles)
RimWorld is fairly amusing in this regard. Feel like an overwhelming game at first. But the more your pawns die from user mistakes. The more you learn from your mistakes....or not.

I can't comment on any others you mentioned, but from what I saw the greatest criticism over Telltale's TWD is exactly that your choices DO NOT matter.

I remember seeing a video where a choice was presented and both paths taken. The illusion that there was a difference is shattered within 5 seconds.

...weird recommendations that are good games with different endings in which choices matter, but they're tied on how you play or what you do in the game - actions - not necessarily choices based in text communication:

Hollow Knight
Sundered
Undertale.

There's different variations on the standard ending, depending on who you dp and don't kill.

Sometimes, your choices can have small effects on later dialog. I believe there's even special dialog for the case where you kill a certain boss, then quit without saving, then reload.

And, of course, there's the special pacifist and genocide route. (Pacifist is similar to the neutral path until the end, while genocide drastically changes the entire game.)

I'm somewhat hesitant about recommending it as a game, since the ball-game that the main challenge in the game was not quite my cup of tea, and I was struggling with the controls. But, despite never playing any sports games, and fighting the game most of the way, I still ended up doing fairly well at it all, getting one of the best possible endings. So - if I can do it, you probably can as well. And despite the above, when I finished the game, I was actually tempted to start a new game immediately to explore some of the possible other paths I could've taken.
Some games that come to my mind right now:

1) Risen 1 (The ending is still the same but choices affect which faction you join and chapters 2-4 If I remember correctly are all very different)

2) Spec Ops: The LIne (Choices will affect the ending)

The actions you take, or choose not to take, during gameplay will lead to one of multiple endings.
Before buying the game you might want to read the enhanced edition release thread to be sure that you are playing the kind of version that you want to play.


Late Shift.

There are multiple endings in the game, and what you choose will make a big difference on who wins, loses, lives, dies and so on.

If you feel lazy, you can just sit back and watch it as a movie. Whenever you need to choose something, if you take longer than few seconds, the game will choose something for you.
