I've just started Cognition: Erica Reed. There's a graphic fight real time puzzle fight, which is pretty shocking. I really recomend this one, it's a detective 6th sense serial killer type and similar in ways to Heavy Rain but with more traditional P&C gameplay.

Oddly, the one that came immediately to mind hasn't been mentioned yet, although the game has (mockingly, deservedly :D).
King Graham's battle with Mordack at the end of King's Quest V. As bad as the rest of the game may be considered today, I'd say that was still a great bit!
